Serum Insulin like Growth Factor Binding Protein-3 and Insulin like Growth Factor- I in Children with Growth Hormone Deficiency and Normal Variant Short Stature


Abstract
Investigation of growth hormone (GH) secretion status in children with short stature is based on pharmacological and physiological tests. There is still some debate as to what the most sensitive and suitable test in the diagnosis of GH deficiency
(GHD)
is. Therefore, attempts were made to find a parameter that reflects the GH secretory status by singie measurements.
Recently, some reports proposed that the insulin like growth factor-I (IGF- I) and/or IGF binding protein-3 (IGFBP-3) were an excellent screening parameter and is highly informative for GHD.
To investigate the possible value of IGFBP-3 and/ IGF- I as a diagnostic marker for GHD, we evaluated serum IGFBP-3 and IGF-I concentration by the radioimmunoassay method in 54 subjects, aged 8-10 years. All subjects were divided into three
groups
(18
children with GHD by conventional criteria, 18 normal variant short statured children, and 18 normal healthy children).
@ES The results were as follows :
@EN 1. The mean serum concentration of IGFBP-3 in GHD was significantly lower than that in normal variant short short stature and normal control.
2. The mean serum concentration of IGF- I in GHD was significantly lower than that in normal variant short stature and normal control.
3. Both mean serum concentration of IGFBP-3 and IGF- I in normal variant short stature tend to be lower than those in normal control.
4. All patients in GHD and only 1 of 18 patients in normal variant short stature had IGFBP-3 values below the 2 standard deviation of mean of normal. But 15 of 18 patients in GHD and only 1 of 18 patients in normal variant short stature had IGF-
values
below the 2 standard deviation of mean of normal.
5. Serum IGFBP-3 levels were positively correlated with serum IGF- I levels in each group.
6. Serum IGFBP-3 and IGF- I levels in GHD rose significantly to normal range after the 1 year treatment of GH.
These results suggest that the meauring of serum IGFBP-3 levels is simpler and serves as a parameter of higher accuracy in the diagnosis of GHD.
